Progress Note Contact Report

The Progress Note Contact report produces a report that shows the data that displays on the Clinical Summary tab above the progress note entry field to distinguish different patient interactions with different healthcare professionals.

You can choose the columns you wish to display in the report.

You can create custom reporting fields for this report - see here for more information. These custom fields are displayed as the final columns of the report.

Inclusion Rules

Progress Note recorded within the same Organisation the reporting user is logged into. See Special Requirements for grouping rules.

Exclusion Rules

Nil

Filters

  • Start Date (midnight of that date i.e. 00:00);
  • End Date (midnight of that date i.e. 00:00);
  • User (All, or per user);
  • Status (Patient Status: All, Active, Inactive).

Special Requirements

The report will group records (per row) when ALL of the following data is the SAME per Progress Note:

  • Organisation: Organisation
  • Progress Note: Patient ID;
  • Progress Note: User ID;
  • Person Contact: Type;
  • Person Contact: Mode;
  • Person Contact: Location;
  • Person Contact: Program;
  • Person Contact: Reason;
  • Person Contact: Outcome;
  • Person Contact: Initiated By; and
  • Custom Fields: All “custom fields” created by the Organisation

N.B. A new row in the report is created when the patient has a progress note recorded where ANY of the data above is different. In other words, the patient may be displayed in multiple rows if the Progress Note/Person Contact/Custom Field data is different.

N.B. An amendment to progress note does not duplicate records.

Custom Fields Names Progress note custom fields configured by/for your organisation
Date Referred Date of the first referral recorded for org (includes radiology/pathology test referrals)
Date of First Contact Date that the patient record was created/registered
Date of Session One Date of the first Progress Note per grouping. See Special Requirements
Number of Sessions The total number (count) of ALL progress notes, irrespective of date range, using the same grouping of Organisation/Progress Note/Person Contact data as described in Special Requirements
Staff Member User that recorded the progress note
Active Patient's current active/Inactive status
Inactive Date Date patient was made inactive
Number of Sessions in Timeframe The total number (count) of progress notes within the date range parameter using the same grouping of Organisation/Progress Note/Person Contact data as described in Special Requirements
Date of First Session in Timeframe Date/Time of the first progress note within the date range parameter using the same Organisation/Progress Note/Person Contact data as described in Special Requirements
Date of Last Session in Timeframe Date/Time of the last progress note within the date range parameter using the same Organisation/Progress Note/Person Contact data as described in Special Requirements

Comments

Filtering by a single day i.e. 14/06/2018 is effectively running a date parameter search as follows:

  • Start Date: 14/06/2018 00:00
  • End Date: 14/06/2018 00:00

Therefore no records will appear.
To view results for a single day (i.e. 14/06/2018), the user needs to change the End Date as follows:

  • Start Date: 14/06/2018
  • End Date: 15/06/2018.
