Summary

The following outlines some processes specific to Joyce Palmer Health Service.

A guide to General Practitioner Clinical workflow can be found here.

Morning Routine:

  • Check Inbox
    • Read new messages
  • Check assigned tasks
    • Act on and respond to these tasks
  • Check Calendar for appointments

Tasks

Tasks should be checked and completed daily

Filter the tasks for Doctors by clicking on Advanced Filters and click on the drop down arrow at By Group and select Doctors

  • Tasks will be displayed
  • Right mouse click on the Task heading and Click Edit this will bring up the Task information
  • Right click shows other options for managing and closing the task when it is complete.
  • Click Update at the top left of the pop-up window to save changes.

Appointment Routine

  • Change appointment status as required
  • Check patient To-Do list for:
    • Tasks
    • Care Plan Activities that are overdue or due shortly
  • Record notes
  • Ensure the following have been recorded as a minimum:
    • Smoking status
    • Alcohol status
    • Weight
    • Height
    • BP
  • Pathology and Radiology requests made through relevant form found in FORMS
    • Use the import overdue Care Plan Activities button to ensure all require pathology has been requested
  • Letters and referrals written using Letter Writer
  • Create Medicare visit, print and have patient sign it before leaving

Patient Appointment Management

Patients presenting at Reception will be booked into the PHCC Doctors Booked in order of arrival.

Health Workers will complete Observations and an initial “screening” of the patient. When the patient is being seen by the Health Worker, the appointment will display as yellow “with Healthworker” and turn blue when waiting to be seen by a Doctor.

Appointment Status Management

Left click on the Appointment to display details – comments will display at the bottom

Patients requesting a specific Dr will be noted in the comments

Take the next patient from the central Calendar

Move the appointment to your own Calendar by clicking (left click and hold) on the appointment and dragging across to your Calendar or clicking on Appointment Detals and changing the Calendar Name to your allocated Calendar.

Right mouse click & click to see appointment details.

A comment may be visible if patient has requested to see a specific Doctor.

Left click on underlined Patient Name to open the Patient Summary

After Billing is completed change the appointment Status to Attended, Billed (green)

Progress Notes

New Progress Notes

Ensure Presenting Complaint and Reason for Contact is entered Privacy Levels for individual notes can be changed where applicable

Notes will autosave evey 5 seconds (approx) however, click SAVE before leaving the Progress notes (Clinical Summary) Page

Progress Note actions

  • Previous progress notes in the Clinical record are displayed under the main Progress Notes field.
  • Print button allows you to select notes to print in PDF
  • Progress notes can be searched for any word entered in the word based text box.
  • Search function searches specifically what you enter into the field from within the progress notes and from subject / reason for contact
  • Change Show ‘1’ to ‘All’ Notes to see all notes on the right hand side of the screen.

Patient-related correspondence that is received by Admin ie: a fax from a Specialist, Specialist Notes on day of appointment or other letters with patient information will be scanned and sent to the referring or named GP as a secure message.

These can be found in Inbox. Attached documents save in the patient record when the message is associated with a patient.

Tasks

Letter Writer

Referrals

  • To refer the patient internally, direct them to reception to have an appointment booked.
  • To refer to external services (including those that are provided by THHS on-site at JPHS) see below steps
  1. Check the Referrals/Visits Tab to check the current Referrals list for an existing current referral for the patient – if there are no referrals for the service required complete the following:
  2. Click on ‘Messaging – Letter Writer’
  3. From this screen your patient should be appearing or Select a Patient
  4. From the Templates drop down arrow & choose JPHS Referral or if you have your own referral setup type in the name of it
  5. Choose a MMEx user in the Address list (using the magnifying glass) or add manually by typing the service or specialist's name into the search field. A list of possible matches will display. See below for local addressing conventions.
  6. Delete or Add information to the Letter as required
  7. Save Letter
  8. Using Secure Messaging send the Referral to the Visiting Specialist Clinic Co-ordinator
  9. Click Send
  10. This will automatically save the Document to the patient Documents & send the referral to Visiting Specialist Clinic Co-ordinator.

Addressing Conventions

  1. Referrals can be addressed but do not need to be – put the service you are requesting at the top of the referral information
  2. Ophthalmology must be referred to – Ian Reddie
  3. Rheumatology must be referred to - Muriel Snowden

Billing the Patient

From the Patient Summary (Progress Notes) page left click ‘Bill Patient’

This will open a new screen ‘Billing’ Check the information entered Check that Billing Type is set to Bulk Billing Check that the Provider is correct – this will automatically update the Payee Provider Number Service Date and Time will automatically set. This can be changed manually Duration can be changed manually

Selecting Medicare Items

Medicare Billing items are shown in a numerical list

Search for your Medicare item by typing in the applicable number OR typing in the type/name of service and clicking Search. Clicking the information icon will open up MBS explanation code pop-up

  • Check the box to include item 10991
  • Click on the relevant MBS item number. This will populate into the Billing items for the patient.
  • CONTINUE to select ALL the relevant item numbers for the patient for the consult with the relevant provider.
  • If you choose the wrong item number click on Remove to delete the item and repeat the process above to select the correct number
  • When satisfied the items and claim is correct click Confirm
  • When there are no errors, Awaiting DB4 Signature is highlighted in yellow. Click Confirm DB4
  • Click Print DB4
  • Patient and Medical Officer sign the Voucher
  • Reception will Collect the Vouchers during the day for reconciling and storage
