Audiology Assessment

Test Details

At the top of the form you will see the following fields for completion:

  • Date - will autopopulate to today's date. The form may be back-dated, but cannot be forward-dated.
  • Audiologist - search for the Audiologist's name (usually your own) in this field. Where data may be inputted by an MMEx user on behalf of a non-MMEx user, the testing Audiologist should have an entry created in the organisation address book. They will then be available for selection in this list.
  • Location - use this field to record where the screening took place. Create an entry in the address book for each screening location.
  • Type of Environment - describe where the testing is taking place
  • History - a free text field to record relevant notes about the patient's ear health history.

Assessments

The following Specialist Audiology Assessments are include in the form.

  • Otoscopy
  • Audiometry
  • OAE Screening
  • OAE Diagnostic
  • Tympanometry
  • Probe Tone
  • Pure Tone Audiometry
  • Audiogram
  • Speech Audiometry
  • Acoustic Reflex
  • VRA(dBHL) / BOA(dBA)

An Amplification field allows you to record any existing amplifications used by the patient. You may record multiple options.


Surgery allows you to record any planned or completed hearing-related surgeries

Any or all parts of the assessment form can be completed. There is no overall outcome assigned to this assessment form so in the Ear Health and Hearing assessments table, the outcome will display as N/A.

When the assessment is complete, then mark the form as complete by clicking Save and Complete. A form that is saved, but not completed remains in draft (and therefore able to be edited by other users) until it is finalised. A pdf version of the form will also be saved in the patient documents.

Printing

There are two options for printing this form.

  • Audiology Report - a comprehensive summary of the complete assessment with every field included in the print-out.
  • Hearing Assessment Results - a one-page summary of results including Otoscopy, Tympanometry, Acoustic Reflex, Reflex Decay Pure Tone Audiogram and Speech Audiometry

Letter Writer

There is also a new autofill marker that populates a letter writer templates with Audiology Results. The results included in the autofill are:

  • Pure tone Audiometry
  • Speech Audiometry
  • Otoscopy
  • Tympanometry
  • Acoustic Reflex

When creating your letter template, from the Patient autofill menu
select
The tag will be placed into the template where your cursor is positioned.
