Aged Care Links

The Australian Government subsidises many different types of aged care services. These aged care packages and programmes are there to help your patients stay as independent as they can, for as long as they can.
Each programme or package has broad eligibility criteria and an assessment process to determine your patient's needs and eligibility. An assessment process helps to ensure everyone receives the care and support they need.

The following people are eligible for ‘MyAgedCare’ assessment:

  • All Aboriginal and Torres Strait Islander people aged 50 years and over
  • Non-Indigenous people aged 65 years and over

Western Australia

The Western Australian Home and Community Care (HACC) Program is a joint funding initiative of the Commonwealth and WA State Governments which provides basic support services for eligible people of all ages with a disability and their carers to assist them to continue living independently at home.

HACC support is designed to assist people with the greatest need and aims to maximise people’s independence. Your patients do not qualify for HACC support on the basis of their age, but on the level of frailty or disability, and on ability to carry out essential everyday activities.
